Dear Student:

Congratulations! You have been accepted into the RN-BS Doctor of Nursing Practice (DNP) in Nurse Anesthesia program at the University of South Florida’s College of Nursing for the Fall 2018 semester. Please consider this your official notification of acceptance to the USF College of Nursing.

It is essential that you read the information below in its entirety. There are several steps required before you are allowed to register for classes, as well as additional information to prepare you for your first semester in the RN-BS program.

Classes begin on August 20, 2018. Your courses have a mandatory first day check in online. If you do not log in to your courses and complete any required assignments by those deadlines, you will be dropped from your course(s).

Please note:

This RN-BS program is intended to be part-time (6 credits/semester) to ensure the success of our working RN’s. A formal request to go full-time (12 credits/semester Fall or Summer; 9 credits/semester Summer) must be sent to your advisor via e-mail to be approved by the RN-BS Program Director. Please inquire with your advisors for the official process by Friday, July 27th.    
*The Castle Branch requirements (Section 1: Step 3) and additional forms must be completed prior to final approval
  • Consecutive enrollment is expected in the RN-BS program (Summer, Fall and Spring semesters). Students who take more than one consecutive semester may have to re-apply and be readmitted into the program.

    Section 1: Prior to Registration

    Items in this section must be completed by Friday, August 10, 2018.

    You will not be able to register for courses until these items have been completed and verified. You will receive an email notification that your holds have been removed and at that time you will be cleared to register at your assigned registration time. Please note: this is a manual process which could take up to 48 hours after completing the appropriate steps.

    titleStep 1: Complete Intent to Enroll form in SOAR

    To confirm your admission to the College of Nursing, you must complete the Intent to Enroll form via SOAR (https://hsccf.hsc.usf.edu/soar) no later than 5:00 p.m. on Friday, August 10, 2018.

    • To complete this form, you will first log into your SOAR (College of Nursing) application (https://hsccf.hsc.usf.edu/soar).
    • Under the section Welcome, you will see a message saying Congratulations on being ACCEPTED…
    • Below this message will be a link to the Intent to Enroll form
    • If you would like to enroll in the College of Nursing’s Fall 2018 program, click "I Accept Admission" and press "Submit"
      • If you have decided NOT to enroll in College of Nursing’s Fall 2018 program program, click I DECLINE Admission and press "Submit"
      • On the next page, please indicate your ‘reason for declining’ and click "Submit" at the bottom of the page.
    • You will not receive a confirmation email
      • To view your confirmation, find your Fall 2018 ACCEPTED application under the Applications on File section and click on the "Intent to Enroll" button

    If your form is not submitted by 5:00 p.m. on Friday, August 10th, you will forfeit your seat in the program and it will be given to the next eligible applicant.

    All College of Nursing students are required to use a USF Health Email address for correspondence with their advisor, professors, and College of Nursing courses. If you have already created a USF Health Email address, please attempt to log in to ensure it is still active. If not - please contact Health IS.

    titleStep 3: Purchase a Background Check, Drug Screening, & Immunization Verification for the College of Nursing & begin uploading your information

    All College of Nursing students are required to complete a Background Check, Immunization Verification, and Drug Screening for the College of Nursing via CastleBranch.com. This is not the same as the USF Health Form from USF Student Health Services. The Castle Branch requirements are required for all RN-BS students. The USF Health Form cannot be used in lieu of Castle Branch.

    To remove the hold, you must purchase the package by August 10th. After you've purchased the package, all Castle Branch requirements must be completed and approved by October 1st  in order to progress to Semester ll. If you are a returning USF RN to BS student, please contact your advisor as you may not need to repurchase the package.

    1. Purchase your CastleBranch account IMMEDIATELY using your USF Health email ($155)
    2. Go to https://portal.castlebranch.com/UD98 
    3. Select Place Order –> College of Nursing –> RN to BSN
    4. Select Code UF17: I am a new student ordering for the first time
    5. Once purchased you will receive instructions from Castle Branch within 48 hours
    6. The urine drug screen must be completed within 72 hours after receiving your instructions
      1. If the drug screening is not completed within 72 hours of purchase, an additional cost will be required as the drug screen package will need to be re-purchased ($38).

    Please contact Castle Branch with any questions regarding the uploading of documentation or requirements for your profile.

    When completing the waiver for the Florida FDLE Fingerprints, please follow these directions:

    The USF College of Nursing Student Academic Compliance Officer Mr. Shawn Afflick (safflick@health.usf.edu) can assist you if the Castle Branch Customer Service cannot resolve an issue. 

    Please note that this is not a one time process. You, the student, must monitor all requirements and keep them up to date during the time of your enrollment in The College of Nursing. Failure to keep the programmatic and clinical requirements up to date will result in a HOLD on your course registration and/or clinical placement.

    titleStep 4: Complete Mandatory College of Nursing Online Orientation & Quiz

    The College of Nursing requires new students to complete a mandatory Online Orientation and quiz. Through this orientation, you will become acquainted with program and college policies.

    • The Online Orientation satisfies the USF Orientation requirement. You do not have to attend an on campus transfer orientation but you are more than welcome to sign up if you wish.

    titleStep 5: Review personalized program plan in DegreeWorks

    Your individual program plan and degree audit (showing outstanding FKL/State Mandated graduation requirements) can be accessed through USF’s online degree audit system, DegreeWorks. You can access DegreeWorks through OASIS (http://oasis.usf.edu/) using your NetID and password.

    Items to note when reviewing DegreeWorks:

    • The Planner tab in DegreeWorks will show a split screen; one screen showing the full degree audit (items in green are complete; items in red are outstanding) and one screen showing your Nursing degree plan for your entire program.
      • RN Nursing coursework is intended to be taken in the order listed in your program plan. If you register for a Nursing course outside of your program plan without pre-approval, you may be dropped from that course.
      • Statistics (and its pre-requisites) must be taken before Semester 3
      • The Capstone and Writing Intensive USF Exit Requirements are built in to your Nursing coursework and will be covered by core Nursing classes.
      • A Portfolio requirement is built into your Nursing coursework and will be graded at the end of NUR4634C in your last semester.
      • Most USF FKL/General Education requirements that you may need can be taken at any time during your program and are not listed under specific semesters on your degree plan.
    • USF Undergraduate Admissions has taken into consideration transfer credit received from your previous institutions. However, DegreeWorks sometimes requires manual application of those credits to the College of Nursing requirements, so please let us know if you see anything missing that you think you have taken.
    • If you attended an ASN school that granted credits based on the quarter system - please schedule an appointment with your Advisor as soon as possible to ensure you will meet the final credit requirements
    • Note that your ASN Nursing courses have not been counted for credit - if you are in need of additional credit in DegreeWorks in excess of the 30 credits in nursing and any other outstanding requirements you have, we will count your nursing courses for credit to cover the outstanding value. This will happen in your first semester once you're enrolled.

    titleStep 6: Determine if you need to complete and return the USF Health Form from Student Health Services (optional depending on schedule)

    The USF Health Form from Student Health Services is not the same as the Castle Branch Profile immunization requirements and cannot be used in-lieu of completing the Castle Branch immunization requirements.

    • The USF Health Form is required ONLY if you plan on attending on-campus courses for any required FKL, Foreign Language, or State Mandated requirements.
    • The hold for the USF health form will always show on your student account unless you complete the above form. If you choose to only take online courses, this hold should never prevent you from registering for your courses.
    • If you plan to take on-campus classes, the form must be completed by a licensed ARNP or Physician. Office stamps and/or additional attached documentation will not be accepted in lieu of a Provider’s signature and license number. Immunization records, along with your signed USF Medical History form, can be scanned and submitted electronically on the Student Health Services website - http://www.usf.edu/student-affairs/student-health-services/services/untitled.aspx

    Once you have completed the steps in Section I and received email notification that the holds have been removed from your account, you are able to register for your courses.

    If you are seeking tuition reimbursement though your employer be sure to confirm policies and requirements prior to registration!

    titleStep 2: Obtain USF Photo ID Card

    All new USF students should obtain a USF Photo ID before the start of the Fall 2018 semester.

    You will need $15.00 (cash or check), a photo ID, and your USF ID number.
    You can order online via their website here HERE or visit their office on campus.

    titleStep 3: Complete HIPAA Certification

    RN-BS students are required to complete the HIPAA certification before August 20, 2018. 

    • Access the website https://learn.health.usf.edu/
    • Log in using your HSCNET/USF HEALTH ID and password.
      • On the homepage, find the drop down box that reads “Select Course Category”
        • Within the drop down, select USF Health Training
      • Click “USF Health Professional Integrity & Compliance – Required Training” to begin your certification

      You must complete the five modules:

      • Medicare Parts C and D General Compliance
      • Combating Medicare Parts C and D Fraud, Waste, and Abuse
      • Professional Integrity Program Handbook
      • HIPPA Compliance
      • Computer and Information Security
    • After you have completed HIPAA training, be sure to print a copy of your certificate (USF health Required Training Completion Certificate) this must be uploaded to your Castle Branch Account.
    • NOTE: Please disable any pop-up blockers before attempting the modules. In addition, make sure to follow all on-screen instructions when exiting the modules. Incorrectly exiting the modules may result in not receiving full completion credit for your attempt. If you need help at any time, give USF HEALTH IS a call at 813.974.6288.

    titleStep 4: Ordering Textbooks

    Textbooks for online classes may be purchased by contacting the USF Health Science Center Bookstore. The bookstore will ship your books via UPS on the same day you place your order IF you place the order before 10:00am.

    Instructions on how to identify your course textbooks for the College of Nursing:

    • Click the “Shop Now” link
    • Enter your course information in the drop-down menu on the right
      • If you do not know certain details of your course (number or section) confirm your registration info in OASIS
    • Add your courses to a list
    • View Textbook List on the left
    • If no textbooks are listed, it may mean your instructor has not added them yet with the bookstore. Contact your instructor for more information.

    titleStep 5: Review the Computer Requirements, Writing Expectations and Tips for Academic Success documents

    The RN-BS program courses are 100% online (not including the service learning hours) and require daily access to the online classroom (Canvas) and email. 

    All USF College of Nursing students are required to have a laptop/notebook computer with wireless internet connection. Students are also required to have a compatible webcam and microphone; the webcam and microphone should either be built into your notebook or be a portable model. Specific information on requirements can be found here. Students in all nursing courses are expected to meet these requirements to successfully participate in their courses. Failure to meet these technology standards may result in inability to complete course or program requirements. It is the student's responsibility to ensure all requirements are met prior to the start of the semester. The USF Computer Store offers laptop "bundles" and discounted software for registered students. For more information, visit http://www.computerstore.usf.edu.

    titleStep 5: Review Service Learning information for Semester II

    Service learning hours are associated with several of the core courses. In collaboration with course faculty and mentors/preceptors, students spend time with leaders in the community while completing projects related to the course. During the service learning hours, students will apply principles, theories, and concepts which are learned in the core courses. Students may do hours at their place of employment if approved by the organization. If students are not working, we will work with you to find placement. Projects are required to complete these courses. Additionally, all requirements in your Castle Branch must be completed and approved prior to the start of the semester in which you are taking these courses.

    *You will be asked to complete a form in your first semester.

    Three Service Learning Courses/Simulation courses:

    • NUR 4828C - Foundations of Nursing Healthcare Leadership & Management
      • The project for this course focuses on working with various managers and leaders to do develop an “organizational culture assessment”
      • Requires precepted hours to be documented in your log book
    • NUR 4169C – EBP for the Baccalaureate Prepared Nurse
      • You will identify a patient outcome that needs to be improved, using evidence to support your assumption. You will develop a PICOT, synthesis of the literature, and a plan for rollout. Your work in this course will be simulated Service Learning hours.
    • NUR 4634C – Population Health
      • The student will assess a specific population within a community where your hours will be completed

    Please keep in mind; it is your responsibility to please notify us if your employment has changed since filling out your SOAR application.

    Students who do not have an approved work location for Service Learning hours will be placed in the Tampa Bay on a space available basis.

    The College of Nursing is unable to guarantee a specific time/schedule or specific preceptor availability for students requiring placement. The student must be able to complete their hours when the preceptor is available to them. Generally, there are no weekend and evening hours available. These hours are intended to be done outside of your normal work hours.

    If you plan to move out of state during your RN to BS program you must contact your advisor ASAP. Service Learning hours must be completed in the state of Florida.

    Congratulations! You have been recommended for admission to the Doctor of Nursing Practice (DNP) in Nurse Anesthesiology program at the University of South Florida College of Nursing.

    The Doctor of Nursing Practice (DNP) in Nurse Anesthesiology program at USF incorporates many cutting-edge technologies, including human anesthesia simulation, while providing a fully integrated learning environment for students. The classroom and clinical setting are designed to achieve integration of the knowledge, skills and ability necessary for the development of competent and proficient Certified Registered Nurse Anesthetists (CRNAs).

    The DNP in Nurse Anesthesiology program has teamed up with the Center for Advanced Medical Learning and Simulation (CAMLS) to provide a state-of-the-art simulation experience, which is integrated into the curriculum. CAMLS is part of the USF Health Simulation Consortium, which has received Level I accreditation as a comprehensive education center from the American College of Surgeons (ACS). The program is a challenging, full-time academic and clinical endeavor.

    Certification: All DNP in Nurse Anesthesiology program graduates are prepared to sit for the National Certification Exam (NCE) administered/tracked by the National Board of Certification & Recertification for Nurse Anesthetists (NBCRNA).

    The Nurse Anesthesiology program is nine consecutive semesters: Summer 2024, Fall 2024, Spring 2025, Summer 2025, Fall 2025, Spring 2026, Summer 2026, Fall 2026, and Spring 2027.  You will follow a lock-step academic program plan and have an anticipated graduation date of Spring (May) 2026. You will be accessed the Programmatic Graduate Tuition Rate for the DNP in Nurse Anesthesiology Program.  Additional program costs will be provided at the mandatory Nurse Anesthesiology Mandatory Orientation on Friday, March 22, 2024 from 10:00 am - 5:00 pm.

    titleStep 1: Purchase your Castle Branch Account

    All students entering the USF College of Nursing are required to open a CastleBranch profile online. The current cost is $181.99. This profile must always be kept current to remain in compliance with the College of Nursing and clinical facilities. Failure to do so may result in dismissal from the program or delay in graduation.

    • You will purchase CastleBranch using an in-state or out-of-state package code. 
      • If you are residing in the state of Florida at the time you are completing Castle Branch, use code UE06
      • If you are residing outside of the state of Florida at the time you are completing Castle Branch, use code UE06osfp
      • Important: out-of-state is considered where you are physically residing at the time you purchase CastleBranch and not your state of residence.  If you are currently residing out-of-state, you must purchase Castle Branch with the out-of-state code so you can receive instructions for the out-of-state fingerprinting process.
    • Once purchased, you will receive instructions from CastleBranch within 48 hours. Your drug test must be completed within 72 hours of purchase.

    titleStep 2: Complete College of Nursing Compliance Requirements

    All College of Nursing students are required to complete a background check, fingerprints, a drug screening, immunization verification, and verification of health insurance for the College of Nursing via CastleBranch.com. This is not the same as the Medical History Form from USF Student Health Services. The Castle Branch requirements are for all students in the College of Nursing.

Please ensure to review this guide in its entirety, as it contains crucial information and may answer questions on completing College of Nursing compliance requirements. 


    You must complete your background check, drug test, and fingerprints within 30 days after receiving your welcome email from our College of Nursing Enrollment Management Specialist.  Verification of Health Insurance must be completed by May 1, 2024.  All other Castle Branch requirements need to be completed by the first day of classes. Please start the process immediately.


    IMPORTANT: If you have an item that is rejected by Castle Branch, please read their comments on why it was rejected and how to correct.

    titleStep 3: Complete USF Immunization Requirements through Student Health Services

    Per USF Policy 33-002, it is mandatory for USF students to submit all required immunization documentation and sign the official Medical History Form prior to course registration. A hold preventing registration in hybrid and in-person classes will remain on your account until you have fulfilled the following requirements. This hold will not restrict registration in fully online classes.  Therefore, it is recommended you register for your online classes as soon as possible and, if applicable, go back and register for any hybrid or in-person classes you have once the IM hold is removed. 

    Please Note: this is a USF requirement to be completed through Student Health Services before you can register for classes. This process is separate from the College of Nursing's CastleBranch requirements.

    titleStep 4: Secure Program Funding (Registration and Purchasing Books Information Included)

    Secure Funding for Summer and Fall 2024

    Remember to submit your two FAFSA applications (2023-2024 & 2024-2025) before the January 1 priority deadline to be considered for various types of aid possible.

    You will need to submit two FAFSA applications due to your start date. 

    • Complete 2023 - 2024 FAFSA
    • Complete 2024 - 2025 FAFSA - (opens December 1, 2023)

    Many scholarships are needs-based and require a FAFSA.  Therefore, it is recommended you complete a FAFSA regardless of whether or not you anticipate applying for federal loans.

    Questions regarding financial aid and scholarships can be directed to a Financial Aid counselor.

    Scholarship Opportunities:

    USF offers a variety of scholarships designed to help you pay for college. To learn more about the USF scholarships for the 2024-2025 academic year, please visit the scholarship website through the USF Office of Financial Aid after it reopens in November 2023.

    HPSP Scholarship:

    The United States Army offers the F. Edward Hébert Armed Forces Health Professions Scholarship Program (HPSP). Qualifying students receive full program tuition and a $2728 monthly stipend for specific accredited health programs - including Certified Registered Nurse Anesthetist (CRNA). Monthly stipend is subject to change June of 2024. There is no military duty while in school. For more information, please see the attached flyer and contact:

    IMPORTANT: Tuition payment or a university deferment is due on the fifth day of the semester.  Please ensure you have funds or a university-approved deferment in-place to prevent being canceled from classes for non-payment.

    Your Nurse Anesthesiology Academic Program Plan is nine consecutive semesters.  You will use your Academic Program Plan for the duration of your program. Failure to adhere to the Academic Program Plan can delay your graduation date.

    • Review the How to Check Registration Time & Holds document (must log-in to your MyUSF) to verify your registration date and times and review any possible holds on your account.
    • All Nurse Anesthesiology Courses are uniquely permitted. These permits allow you to register for Nurse Anesthesiology courses each semester. Your seat in every class is reserved for you. You will receive an email from your Academic Advisor, Melissa Jay before each semester with registration information.
    • You can view registration holds in your MyUSF: OASIS - Student - Student Records - View Holds.
      • Only the office that placed the hold on your account is authorized to remove the hold.
      • Therefore, if you have questions regarding how to remove a hold, please contact the office that placed a hold.
    • Review the Course Registration Module to learn how to search for course sections, identify the Course Registration Number (CRN) and instructions on how to register for courses in OASIS using the CRN.  
      • Your specific CRN numbers needed to register each semester will be emailed to you by your Academic Advisor, Melissa Jay.

    Registration Tips:

    • It is best to register for all classes at once.  You will receive a "corequisite error" if not simultaneously registering for courses and their corresponding clinicals.
    • If you have a university/Student Health Services Immunization (IM) hold, it will prohibit you from registering in hybrid and in-person classes. You must follow the instructions in "Step 3" to have the hold removed.


    Purchase Textbooks/Course Materials - (After Orientation)

    Once registered and closer to the start of the semester, you will purchase your textbooks and course materials. Please review the instructions on how to find the required textbooks and course materials.

    titleStep 5: Submit Florida Residency Documentation (If Applicable)

    Residency Classifications

    Initial Classification

    When you apply to the university, your initial residency classification is determined by the Office of Admissions based on information you provide with your application.

    The classification is determined by the information provided on your application. Failure to provide all relevant information, including copies of required documents, may lead to an initial classification as an "out-of-state" resident.

    It is your responsibility to check your residency classification when you are admitted to the University of South Florida.

    If you feel your initial classification is in error, you have until the last day of the term of admission to contact the appropriate admissions office and request re-evaluation. Once you have completed your first term, you may only request reclassification for future terms.

    Reclassification

    A Request for Reclassification and additional documentation must be submitted to the Office of the Registrar no later than the fifth day of the first week of the semester for which reclassification is sought. There are no exceptions to this deadline. Requests received after the published deadline will be considered for a future term only, and tuition will be assessed at the non-resident rate for that term. Please note, until a decision has been made on your request, you should consider yourself a non-Florida resident for tuition purposes. It is your responsibility to adhere to all fee payment deadlines for the term. If you are approved for in-state residency after payment is made, you will be refunded the out-of-state portion of your fees.

    If you are submitting a Request for Reclassification for Summer term you must do so by the deadline specified, regardless of when your classes begin. The domicile year for which your request is evaluated is also the same and not depending on your summer class schedule.
