DPT Financial Aid

We are happy you have considered the USF Health Morsani College of Medicine as your next academic endeavor. Please take the time to read all of the information provided. It is a valuable resource for the federal and College financial aid processes and will offer the tools you need for financing your medical education.

It is the intention of the USF Health Morsani College of Medicine Financial Aid Office to help you find a suitable method of funding your medical school education. The purpose of this letter is to help you to begin thinking on the right track regarding financial aid and address important issues for your consideration regarding financial aid.

The USF Health Morsani College of Medicine has a Financial Aid Office that is independent from the central campus USF Financial Aid Office. All aid applications, inquiries, and correspondence must be sent directly to the USF Health Morsani College of Medicine Financial Aid Office:

This is particularly important if you are expecting scholarship funds from any outside agency such as a hometown hospital, church group, etc. Be sure to notify the agency to send award checks to the above address. If paperwork or checks are sent to the central campus Financial Aid Office, it will run the risk of not reaching the Morsani College of Medicine Financial Aid Office at all.

"Financial aid" does not mean grants or scholarships exclusively. Loans (frequently high interest loans), employment, and making an effort to reduce spending all fall under the heading of financial aid. Financial aid officers in medical schools across the country have become more "financial and debt management counselors” than "financial aid counselors". The USF Morsani College of Medicine has limited grant and scholarship funds which are not sufficient to offset the fact that you may need to take out loans to pay for your medical education. There are several loan programs available for DPT students. Our office will help you with the application for those loans, advise you on the "best" loan programs available, and counsel you on how to keep your borrowing to a minimum.

Fortunately, we still have a low tuition compared to other medical schools in this country. Even with that said our students HAVE HAD TO: 1) make adjustments in their life styles to allow for serious cuts in their spending; 2) work during the summers and save money to help pay for their education; 3) receive financial assistance from parents, grandparents, etc.; and 4) have their spouse obtain employment to help with expenses. We call this "self-help" and we do not think it is asking too much for each student to provide some "self-help" in order to obtain their medical education.

Paying for your medical education is an issue that requires immediate and serious attention from you and your entire family. The financial aid funds available for DPT students are almost exclusively federal funds and are therefore subject to rather strict federal guidelines with regard to student qualification. This office must adhere to and enforce the federal guidelines. The USF Health Morsani College of Medicine has adopted most of the same federal policies for awarding their limited institutional funds. The policies are based on the assumption that your medical education is a priority for you and your family; therefore, paying for your medical education will likewise be a priority. We are very sensitive to the fact that professional students would like to be "independent" of their families; however, you cannot be "independent" if you are simply transferring your financial dependence from your family to the Federal Government, or to this school. We also understand that students do not wish to burden their families, even when they are willing and able to help financially. Remember the loans you borrow now must be repaid. By making every effort to obtain family support, living on a frugal budget, and keeping your borrowing to a minimum you will be rewarded by having low monthly payments once your repayment begins. Keep in mind for every one dollar borrowed now may result in three dollars being repaid.

You will be receiving correspondence from the USF Health Morsani College of Medicine Student Affairs Office under separate cover, which will provide you with information about the curriculum, student health issues, tuition, and other items of interest.

The Financial Aid Application Process: Instructions for DPT Students

The following forms must be completed in order to determine your eligibility for USF Health Morsani College of Medicine loans and scholarships. Your application for financial aid will not be processed until all documents have been received by the USF Health Morsani College of Medicine Financial Aid Office.

If you are applying for:

  • Unsubsidized Federal Direct Loan (UFDL)

  • Federal Graduate/Professional PLUS Loan (Grad PLUS)

Please complete the following steps.

 

  1. Free Application for Federal Student Aid (FAFSA):

    A FAFSA must be completed for all financial aid programs. This can be done on line:

    Request a PIN ID

    NOTE TO ALL APPLICANTS:

    a.) If you have not completed your 2012 tax return you may provide estimated information on the FAFSA to meet the deadline. As soon as your 2012 tax return is completed you will need to update your FAFSA online. Financial aid awards will be adjusted if needed.

    b.) Please complete the FAFSA according to the instructions that are provided. An incorrect or incomplete application will delay the financial aid process.

    c.) When identifying USF Health Morsani College of Medicine as the school you will be attending use E00568 as the school ID. This will insure that the USF Health Morsani College of Medicine will receive your FAFSA data.

  2. Financial Aid Award Notice – You will be notified by e-mail when your financial aid eligibility is determined

  3. Federal Loan Master Promissory Note(s) – Instructions will be provided about how to complete the loan application & promissory note when a loan is accepted

  4. Verification - If your file is selected for “Verification” you will be notified by email and must respond quickly. Financial aid files pending verification are considered incomplete.

 COMMUNICATION

E-mail is our primary means of communication with students prior to matriculation. We may also send mailings via the U.S. postal service. DPT students must communicate e-mail or U.S. mail address changes to the School of Physical Therapy & Rehabilitation Sciences (Mr. Matt Smith – msmith4@health.usf.edu). 

 DPT FALL PREVIEW

Students matriculating in the DPT Class of 2018 must attend DPT Fall Preview, which takes place Tuesday,July 28, through Thursday, July 30, 2015. Attendance and participation in all events during this period (typically 8:00 AM – 5:00 PM) is mandatory. During this period, students meet the faculty and staff, receive handbooks and tools, and learn about the curriculum, in addition to completing Basic Life Support training. Student ambassadors are on hand to answer questions and provide information about opportunities available in the SPTRS community. There is also a Financial Aid Workshop for those accepting financial aid, and faculty members present course overviews for the Physical Therapy courses in the first-year curriculum. Fall Preview concludes on Thursday, July 30, 2015, with DPT Family Day, occurring from 9:00 AM – 1:00 PM at the School. Students are invited to bring whomever they consider to be their support team to tour the School and learn about the DPT program.

 REGISTRATION AND ORIENTATION

Registration and Orientation Day for students in the Morsani College of Medicine (including DPT Program, Core MD Program, and SELECT MD Program) will occur Friday, July 31, 2015 in the LVHN Learning Center, Room MDA 1209. We will begin with a Welcome Breakfast from 7:30 – 8:15 AM, followed by a series of short presentations and registration activities. It is essential for all new medical and physical therapy students to report on July 31, 2015 and remain in attendance throughout the entire day until 5:00 PM. Students must dress professionally (i.e., men should wear a shirt and tie; jacket is not necessary). Formal portraits (head shots) will be made and assembled for the class composite. All students will be fitted for their white coat at Orientation. (Coats purchased through the COMPAC website are different and are used for a distinct purpose.)

 HOUSING AND ROOMMATE LIST

On-campus dormitories at USF are not-well suited for MCOM professional degree program (DPT) students due to differences in the academic calendars. We recommend students obtain nearby housing off-campus and room with another medical/physical therapy student. USF Health has affiliated with a housing service to assist students looking for housing in the Tampa Bay area. Visit USF Health Real Estate with Honors for more information. 

 FINANCIAL AID

Financial Aid information and instructions were e-mailed to you under separate cover. Award notices will be sent to you as your application packets are completed.  DPT STUDENT TUITION, FEES AND OTHER EXPENSES

Various fees will be collected at Orientation on July 31, 2015, so be sure to bring your checkbook. We cannot accept cash, credit or debit cards. Tuition and fees for the 2015-2016 academic year are ESTIMATED to be $22,100 for in-state students and $29,000 for out-of-state students. The yearly amount is split into 2 installments. The first installment, an estimated amount pending approval, and the due date will be determined by the Florida Board of Governors. The second installment will be due in January 2016. Your tuition assessment will be available online approximately 30 days prior to the start of classes. An e-mail with your login information will be sent at that time by Ms. Marrissa Cook, Morsani College of Medicine Registrar. Other approximate expenses during the first few weeks of school include: Books, $1000; ResponseCard RF LCD “clicker,” $50, and miscellaneous supplies, $400.

 PROFESSIONS OF HEALTH COURSE

The USF Doctor of Medicine and Doctor of Physical Therapy course work will begin with a week-long introduction to teamwork and interprofessional education. Twenty-first century health care is not delivered by individuals but by teams, and the first week will give you the chance to work with a group of students from multiple professions on multiple tasks, both academic and experiential (with patients and healthcare providers). In performing these tasks, we hope you will begin your studies with an appreciation for how health care depends on many partners, and how your awareness of who your partners are and what they contribute to patient care will be critical to your success as a physician or physical therapist. It should also be a lot of fun!

 DPT STUDENT COMMITMENT TO PROFESSIONALISM CEREMONY

Students in the DPT class of 2018 will participate in Commitment to Professionalism (White Coat) Ceremony on the evening of October 15, 2015. You will receive additional information regarding the ceremony at a later date, as your family and friends may wish to attend this event.

 TECHNICAL STANDARDS

DPT students were provided a copy of the technical standards with their acceptance letter. Students must advise the Office of Student Affairs in writing of any accommodations that may be needed to assist in accomplishing the requirements specified in the technical standards.

 EXPECTED COMPUTER COMPETENCIES

Students must be able to demonstrate basic computer skills upon entry into medical school including the ability to:

  1. Launch a computer application.
  2. Save work to a computer file.
  3. Print a file.
  4. Copy a file for use on another computer.
  5. Use a standard word processing program to create and edit a formatted document using tables and graphics.
  6. Use e-mail effectively, including proper etiquette.
  7. Access and use the Internet.
  8. Create a PowerPoint presentation.
  9. Students not proficient in these skills are invited to participate in any of the training workshops offered by USF Health Information Services.
 MORSANI COLLEGE OF MEDICINE PEER ADVISORY COMMITTEE

The Morsani College of Medicine Peer Advisory Committee (COMPAC) is a group of twelve second-year MD students and two second-year DPT students dedicated to welcoming new MD and DPT students into the USF Morsani College of Medicine. You will receive communications from them throughout the summer about planned events.

 3. Obtain a USF DPT ID card

Apply for a USF Medical/Physical Therapy Student Badge/ID card at the USF Card Center located in the Marshall Student Center, Room 1505. Hours are Monday-Thursday, 9:00am-6:00pm, and Friday, 9:00am-5:00pm. For more information please call (813) 974-2357.

 4. Submit residency forms and documentation

The completed Florida Residency and Reclassification Affidavit must be submitted directly to Matt Smith (not to the MHA address given on the form), along with copies of the required documentation. It is of utmost importance that the instructions be followed carefully, as submitting the document incorrectly will hinder you from being able to complete additional next steps. Note: Effective July 1, 2009, non-resident students who come to Florida and enroll in a Florida post-secondary educational institution will not meet the Florida residency requirement for in-state tuition, regardless of the length of time enrolled. For tuition purposes, you must obtain Florida residency at least 12 months prior to your first day of class.

 5. Secure health insurance

The Morsani College of Medicine has the following four (4) health related requirements that must be met prior to matriculation:

  1. Students must have health insurance in effect at all times during enrollment at USF MCOM. During orientation you will be required to sign a statement certifying you have health insurance coverage.  Annual recertification is required throughout your enrollment. Information on the health insurance policy offered by USF will be available at orientation for students who do not have current health insurance.
  2. DPT students do not need disability insurance.
 6. Submit physical exam verification form, immunization form, and documentation
  1. Students must undergo a physical examination prior to starting school. Complete the Physical Exam Verification form (last sheet in the packet) and take it to your health care provider for completion of their section. Return the form to Linda Lennerth, RN, Associate Director, Medical Health Administration (address on form) as soon as possible, but no later than July 1, 2015.
  2. Students must complete and provide documented evidence of immunity to specified communicable diseases listed on the Communicable Disease Prevention Certification form. All immunity documentation must have the student’s name clearly identified on every page/sheet; particularly when submitting documents in a “book” format. A list of acceptable documentation is provided on the Communicable Disease Prevention Certification form. Return the form and documentation to Linda Lennerth, RN, Associate Director, Medical Health Administration (address on form) as soon as possible, but no later than July 1, 2014.  All immunizations must be up to date at the time of matriculation on July 28, 2015. Students are not permitted to begin classes or to work in any of our hospitals or clinics without providing the required evidence of physical examination and documented immunity to the diseases listed.

 7. Purchase textbooks & Stethoscope

The textbook list for Year 1 students will be distributed in July 2015. In addition to textbooks, students must purchase a stethoscope that meets the following minimum criteria for use during all three years of the program:

•     adult dual head (bell & diaphragm) – not electronic

•     short tubing (about 27 inches) – any longer length sounds too quiet, less distinct

•     tubing should be flexible for folding into lab coat pocket

We recommend that you purchase a high-quality stethoscope that will serve you throughout your career, such as the Littman Classic II SE, which costs about $70-80. Many students receive these as undergraduate Commencement gifts; but if desired, the class can work together to obtain a group discount from medical suppliers.

 8. Purchase a ResponseCard RF LCD clicker
All students must purchase a ResponseCard RF LCD from Turning Technologies at the USF Health Bookstore for a cost of approximately $50 (including tax).
 9. Purchase parking decal

A valid USF Parking Permit is required on all vehicles parked on campus. Morsani College of Medicine students must purchase a Non-resident Student Permit.  If you are unable to secure your Non-resident Student Permit prior to Orientation on July 31, 2015 make sure you purchase a Visitor Permit on the morning of August 31st as explained below.

  1. Non-resident Student Permit – Online sales for 2015-2016 will be available starting on July 15, 2015.  If you already have a parking account you can log into myUSF with your NetID, and under the Services tab click on the Parking link to access your account. If you do not have a parking account, please visit Parking & Transportation Services to Create an Account. At the time of purchase you will be able to print out a temporary parking permit to use until the permanent permit arrives via U.S. mail.  Allow one week for delivery. Temporary permits are valid for 14 days. For more information please call (813) 974-3990. You are strongly advised to purchase your permit online prior to the start of school.
  2. Visitor Permit – This is available from Parking Pay Stations located in Lot 19 on USF Magnolia Drive by the Moffitt Child Care Center. The cost is $5/day plus tax and payable by Visa, MasterCard or Discovery only.

Note:  Students should purchase an annual permit, not a semester permit. An annual permit is effective for a 12-month period from August 2015 to August 2016. The effective dates for a semester permit do not extend through the end of the first-year curriculum in 2016. A semester permit would not allow you to park on campus during the final 2 to 3 weeks of school.

The Non-resident Student Permit allows you to park in any lot/garage designated “S” (Non-resident Student).  Lot 32 on USF Hawthorn Drive is the closest lot to the medical school with over 450 spaces available on a first-come, first-serve basis. Lot 33T on USF Health Drive has over 100 “S” spaces available on a first-come, first-serve basis.  Overflow student parking is accommodated in Lot 19 on USF Hawthorn Drive by the Moffitt Child Care Center and in the Laurel Garage on USF Laurel Drive next to the Morsani Health Center and is serviced by the Bull Runner B route.  The most economical parking permit is the “Y” (Park-n-Ride), and Lot 43 on USF Laurel Drive is the closest “Y” designated parking to USF Health, and is serviced by the Bull Runner B route.

 10. Obtain a laptop computer and printer - requirements

All USF Morsani College of Medicine Doctor of Physical Therapy (DPT) students are required to own a laptop computer; it is highly recommended that you also purchase a color laser printer for instructional purposes. Students must also have a high-speed internet connection at their residence in order to access online course information.

Students must have your network computer available for use on Wednesday, July 29, 2015. Specific requirements will be posted on the MCOM website by June 30, 2015. USF Health Information Systems will assist with configuring your laptop computer and software upon request; they may be reached at (813) 974-6288.

Note:  Students will use SofTest software for exam taking.  At this time SofTest is not compatible with Windows 10.  Do not upgrade your computer to the newest operating system until you are notified that it is compatible with SofTest.  

--- Competencies ---

 11. Prepare to demonstrate medical terminology competency

Students in the USF Health SPTRS Doctor of Physical Therapy program must possess basic competence in medical terminology. Students may achieve competence in a number of ways. We recommend the free online “Medical Terminology” course offered through Des Moines University (https://www.dmu.edu/medterms/). Note: it is not necessary to present the course’s Certificate of Completion to SPTRS. Competence will be assessed for all entering students using an electronic test during Year 1 Fall Preview (July 28-30, 2015); a minimum score of 85% is required. Because of the variability in curricula, standards, requirements and types of assessments used at different institutions, we are not able to accept completion of any other course or assessments in lieu of this SPTRS test requirement. Students whose performance on the initial test falls below 85% will be permitted limited opportunity to review the material and re-take the assessment. Competency is a precondition for continued matriculation and failure to demonstrate competency within the first month of classes will result in dismissal.

 12. Obtain a Name Badge

The Office of Student Affairs will make name badges for each new student, which you will be required to wear daily during the first 2 weeks of school. Name badges will contain your official name as reflected in your BANNER registration. If you prefer an alternate name (e.g., middle name or nickname) and wish to have that name printed on the name tag in lieu of your first name, please email Ms. Meghan Connery (mconner1@health.usf.edu ) no later than July 10, 2015. Changes will not be made once school begins on August 3, 2015.
