How To Request Degree Verification (Graduate Program):
Step 1 - Please obtain the following forms:
- Verification form (with graduate’s information completed)
- Signed release of information authorization
Step 2 - Send your completed forms to con-gradadvising@usf.edu. Note: Please include your full contact information, including pervious names used, and it would also be helpful to include your U number for quick processing.
Step 3 - Your form will be processed in the order in which it was received, which typically takes 3-4 weeks, and forward the completed forms to one of the following, as requested:
- Florida Board of Nursing
- Other state’s Board of Nursing
- Potential/current employer

How To Request Degree Verification (Undergraduate Program):
Step 1 - Please obtain the following forms:
Step 2- Request Official Transcripts indicating that you have a Supplemental Form:
Step 3 - The Registrars Office will notify the College of Nursing to complete the supplemental form and will be sent back to Registrar's Office to include with your official transcript.
Requests are processed in the order in which it was received, which typically takes 3-4 weeks, and will be provided to the Registrars Office to include with your official transcript.
