How To Request Degree Verification:

Please obtain the following forms and send via mail, fax or email:

  • Verification form (with graduate’s information completed)
  • Signed release of information authorization

Note: Please include your full contact information in all correspondence.

We will process your request within five to seven business days and forward the completed forms to one of the following, as requested:

  • Florida Board of Nursing
  • Other state’s Board of Nursing
