As you plan your 4th year electives – including any “away” electives not at USF-LV or Tampa – USF-Lehigh Valley’s Employee/Student Health Services Office is here to help.
Please note that the below process is similar to the process as explained by Linda Lennerth for students in Tampa, however, this email contains information specific to students at USF-Lehigh Valley.
- You should have received/will receive from the Health Office at USF-Tampa an email that contains a copy of your immunization report that includes all the information on-file at the MHA office in Tampa, as well as any information that may be on file at USF-Lehigh Valley. As of last week, all records at USF-Tampa are complete and include immunization information from USF-Lehigh Valley. If you have not received a personal email with your immunization report, contact Linda Lennerth at llennert@health.usf.edu.
- Review and compare your immunization report with the requirements for the facility where you are applying for an externship. The requirements in some facilities may differ from the USF requirements and you may need to have additional testing done such as antibody titers.
- Complete any additional requirements (if applicable), including lab testing, TB screening, and/or fit-testing and obtain a copy of the verifications. Comparable to USF-Tampa, USF-Lehigh Valley’s Employee/Student Health office can only provide TB screening and flu shots. In addition, our Infection Control and Prevention Department performs annual fit testing. There is no charge to you for these services through our office. Any other requirements are your responsibility to have completed at either your PCP office or at a location of your choosing, at your expense. - Complete all of the on-line Safety training modules (5 Total) located on the USF LEARN website https://learn.health.usf.edu/login/index.php. Log in, click on “USF Health Training” and then “Role Based Training”. The 5 required training modules are located in the “Safety: Back to Basics” folder. Make sure you complete all 5 modules.

- Fit-testing will be offered to students during Prologue 4 at the end of May. To be fit-tested, you must complete and bring with you the attached “Medical Questionnaire for Continued Respirator Use.” This needs to be completed prior to fit testing. If you need fit - testing to fulfill an externship requirement prior to May, please contact me directly at Angela.Davis@lvhn.org.
