As you plan your 4th year  year electives – including any “away” electives not at USF-LV or Tampa – USF-Lehigh Valley’s Employee/Student Health Services Office is here to help.

  1. Review and compare your immunization report with the requirements for the facility where you are applying for an externship. The requirements in some facilities may differ from the USF requirements and you may need to have additional testing done such as antibody titers.
  2. Complete any additional requirements (if applicable), including lab testing, TB screening, and/or fit-testing and obtain a copy of the verificationsComparable to USF-Tampa, USF-Lehigh Valley’s Employee/Student Health office can only provide TB screening and flu shots. In addition, our Infection Control and Prevention Department performs annual fit testing. There is no charge to you for these services through our office. Any other requirements are your responsibility to have completed at either your PCP office or at a location of your choosing, at your expenseClick here for a listing of suggested locations http://www.lvhn.org/facilities_directions/walk_in_care We strongly suggest you call these locations first to ensure they can assist you and to ask about pricing.
  3. Complete all of the on-line Safety training modules (5 Total) located on the USF LEARN website https://learn.health.usf.edu/login/index.php.  Log in, click on “USF Health Training” and then “Role Based Training”.  The 5 required training modules are located in the “Safety: Back to Basics” folder.  Make sure you complete all 5 modules.

Note: TB and fit-testing are annual requirements for students at LVHN. For your convenience, at no charge to you, the USF-Lehigh Valley Student/Employee Health Office can help you complete your TB screening (this will consist of a TST if previously negative TST/QFT, a QFT if the externship requires a QFT, or a sign/symptom screen if history of documented positive  QFT), also obtain required signatures. Infection Control and Prevention performs annual fit testing.

To obtain a required signature for an externship form:

  1. If you will be applying for an externship that requires a signature verifying that you meet all immunization requirements, you should schedule an appointment with USF-LV’s Employee/Student Health office. Let us know how many applications you will have so we can allot enough time to complete the review(s).  The appointment should be scheduled AFTER you have completed your annual TB screening and after you have completed any other additional testing required for the externship. Bring the documentation of the results of any additional testing that was required and performed  outside of USF student health/Employee Health with you to the appointment.  Employee health cannot sign the document unless you have provided the  results of additional testing done outside of USF/LVHN Employee Health. Contact info and hours are attached. WE CANNOT REVIEW EXTERNSHIP APPLICATIONS ON A WALK-IN BASIS.
  2. Complete the Externship application immunization form(s) using your USF immunization report emailed to you and bring the completed forms to your appointment.
  3. Employee/Student Health office staff will update your USF record (if applicable), review, stamp and sign your applications for externships at the time of your appointment.

To complete TB testing:

  1. Schedule an appointment with the Employee/Student Health office if you need an updated TB test. Contact info and hours are attached. WE CANNOT GIVE YOU AN UPDATED TB TEST ON A WALK-IN BASIS.

To complete fit-testing:

Lehigh Valley or Tampa, you may be asked by host institutions to provide them with additional testing or titers, based upon a review of history.

Below are the steps to help guide you through completing these additional rotation requirements, if necessary:

  1. SELECT student requests their immunization/test records from Medical Health Administration (USF Tampa) either by email: mha@usf.edu or phone: 813-974-3163
  2. Student compares the requirements of the receiving hospital to the records obtained from USF.
  3. For physicals, additional titers, immunizations or other testing (including drug testing), these can be done by Concentra.
    1. Please call to make an appointment, and identify yourself as a USF student when scheduling.
    2. Walk-in hours are available for titers, immunizations, and drug screenings. Titers can be performed Monday through Friday until 1:30 pm each day.
    3. Students are responsible for all expenses including the cost of vaccine, immunization and/or drug testing fees needed. 
    4. You will be required to pay for charges up front and then submit to your insurance for reimbursement.
    5. You will be responsible for all charges not covered by your insurance.

  4. TB Screening: If the only item needed for the rotation requirements is a TB screen, plus completion of the medical form verified by an HCP signature, Employee Health Services will provide this as a courtesy to you.  Go to Employee Health during WALK-IN hours and have the TB screen done.  TB screening will be done by PPD Mantoux testing.  QFT testing will be provided ONLY when a QFT test is a rotation requirement.  
    If you need your medical requirement form signed by a HCP, leave your forms AND your USF records with the Employee Health Receptionist at that time.  After the TB screen is read/reported, the Employee Health Nurse will complete the form and notify you to pick up the forms.  If only the verification of medical forms is needed, drop off the forms, with your USF records in Employee Health, and you will be called to pick up when completed. 
  5. Fit Testing: Fit-testing will be offered to students in June during Prologue 4 at the end of May. To be fit-tested, you must complete and bring with you the attached “Medical Questionnaire for Continued Respirator Use.” This needs to be  completed prior to fit testing.

  2. Flu Shots: During flu season, Employee Health will provide flu shots to all SELECT students at the SELECT 3 Flu Clinic that is arranged by Student Affairs. Students will also be able to get one at the Employee Health office by registering online. Employee Health does NOT offer walk-in hours for flu shots.
