Thank you for submitting your Experiential Learning Form (Service Learning Form) via Qualtrics. The Clinical Coordinator will be your primary USF College of Nursing contact for securing a placement for you to complete your experiential learning (service learning) hours with your clinical site or hospital. However, this does not necessarily mean the Clinical Coordinator secures your preceptor. This information is determined by the facility, and you can find more details below in the drop downs.
Important to note: You will need to wear a Lab coat & your USF Student ID badge whenever you are within the hospital or facility for your NUR4828C practicum hours. Most hospitals prefer the differentiation of the student role from your employer role. Therefore, you don't have to necessarily buy a USF lab cost; however, you must have a lab coat on and your badge must be visible, also, you must wear different clothing different from your work attire under your lab coat. There are also USF College of Nursing patches in the USF Health Bookstore. |

Step 2: Upon completion of reading the NFRMC – 1 Student Orientation Learning Packet Students must sign and complete the NFRMC-2 Student Orientation Packet Signature Page-- https://usf.box.com/v/NFRMC2.
