Step 1 - Please obtain the following forms:

• Verification form (with graduate’s information completed)

Step 2- Request Official Transcripts indicating that you have a Supplemental Form: 

• Upload the supplemental form to your transcript request

Step 3 - The Registrars Office will notify the College of Nursing to complete the supplemental form and will be sent back to Registrar's Office to include with your official transcript.

Requests are processed in the order in which it was received, which typically takes 3-4 weeks, and will be provided to the Registrars Office to include with your official transcript.
