AFTER OFFICIAL NOTIFICATION FROM USF OFFICE OF ADMISSIONS


Dear Applicant,

Congratulations! You have been recommended for admission to the DNP program of the College of Nursing at the University of South Florida.

Please read the following information carefully, as it includes important action items and deadlines. It is crucial that you follow each step in order.

At this time, please complete the following:

  1. Intent to Enroll survey  
  • DNP Intent to Enroll survey: https://usfhealth.az1.qualtrics.com/jfe/form/SV_3sfm6xj4LdVcXTn 
  • The survey confirms whether you have accepted the offer for admission into the DNP program. The offer of admission will be reviewed and processed by the USF Office of Admissions in the coming weeks, and a formal letter of admission will be sent via postal mail.
  • If your decision changes between now and the first day of classes for Fall 2018, please notify the College of Nursing at nurstudent@health.usf.edu 


    2. Order your CastleBranch package for background checks and clinical immunizations 
  • On the CastleBranch website, https://portal.castlebranch.com/ud98, select “Place Order” and follow the prompts. Be sure to select Place Order > College of Nursing > Graduate Program > DNP > Code UE05: I am a new student ordering for the first time.
  • All parts of your CastleBranch package must be processed and approved by the CastleBranch system no later than October 1, 2018. Students without a complete CastleBranch package will not be eligible to register for second-semester courses.
  • Your package includes levels 1 & 2 background checks, drug test, and clinical immunization requirements.
  • The cost of CastleBranch package is $153.75, due at the time of order. Refunds are not guaranteed if you do not matriculate into the program.

Please complete the following items after you have received your email letter of admittance from the USF Office of Admissions:

  1. Set up USF Student Health Email Account and NET ID
  • Your USF Health email account must be used for all official correspondence with the College of Nursing. Please see the Onboarding Students instructions to establish your account: http://health.usf.edu/is/get-started#students.


  • 2. Complete HIPAA Certification
  • All students are required to complete the HIPAA certification at https://learn.health.usf.edu/.
  • Log in using your HSCNET/USF HEALTH ID and password.
  • On the homepage, find the drop down box that says “Select Course Category”
  • Within the drop-down, select USF Health Training
  • Click “USF Health Professional Integrity & Compliance – Required Training” to begin.
  • You must complete the five modules: Medicare Parts C and D General Compliance; Combating Medicare Parts C and D Fraud, Waste, and Abuse; Professional Integrity Program Handbook; HIPAA Compliance; Computer & Information Security.
  • After you have completed HIPAA training, be sure to print a copy of your certificate (USF Health Required Training Completion Certificate) and upload it to your Castle Branch account. Please also save/print a copy for your own records.

    3. Complete the USF Student Health Services Health Form 
  • Find the form here: http://www.usf.edu/student-affairs/student-health-services/documents/mandatory-imm-form-122014.pdf.
  • Please follow the directions on the form and submit the completed form to USF Student Health Services (not the College of Nursing). The address and fax number are located on the form. You may also submit it online at http://form.jotformpro.com/form/32133984690964.
  • USF Student Health Services places a registration hold on your USF student account until you have completed the form.

    4. University of South Florida Responsible Conduct of Research (RCR) Requirements
  • All new doctoral students at USF must have basic RCR training by completing the Collaborative Institutional Training Initiative (CITI) module most relevant to your program of study. The CITI modules have been designed to introduce researchers to various elements of research conduct ranging from research misconduct to data management to mentoring. As this is a minimum requirement, specific doctoral programs may require training that goes beyond the basic components introduced in this module. Furthermore, you must complete the module in the first semester you are enrolled in a doctoral program as a registration hold will be placed on your account; you will be unable to register for courses in a future semester until you have successfully fulfilled this RCR requirement.     5. Online Orientation: August 3, 2018, from 11 AM to 1 PM
  • The College of Nursing Student Affairs Office will hold an online orientation on August 11, 2018, from 11 AM to 1 PM
  • During this orientation, you will meet College of Nursing faculty, concentration directors, and students in your program. We will also introduce you to the many USF Health campus resources and services.
  • Additional onboarding information, including parking and how to obtain your Student ID card, can be found on the Office of Graduate Studies' webpage at http://www.grad.usf.edu/new_students.php 

    6. Program plans/registration
  • Your academic advisor will contact you in the near future. At that time, you will be provided your program plan and registration information via email. Until then, please continue working through the steps listed above.
