Increasing numbers of externship programs require applicants to have completed at least one recent screening service, such as fingerprinting, criminal background check, or drug screening. Proof of HIPAA/OSHA training may also be requested.

These services are NOT part of your educational record, thus USF Student Affairs and Registrar cannot distribute this information for you. It is your responsibility to maintain copies to mail or upload for visiting applications.

Before beginning clinical rotations or clerkships, many facilities (including the Lehigh Valley Health Network) require fingerprinting and/or drug testing of medical students, while Moffitt Cancer Center requires evidence that you have done so and that you have received the criminal background check. Occasionally, externship programs require Year 4 students to provide documentation for fingerprinting and a criminal background check analysis.

You can access your background check completed through AMCAS when you applied to medical school, in many cases.  To access that you can log in to Certiphi’s Consumer Care website https://consumercare.certiphi.com/ and download your report.  If a more recent background investigation is required by a program, follow the instructions below.  

Fingerprinting and/or Criminal Background Check

Below are step-by-step instructions for accessing Application Station: Student Edition to authorize and pay for a background investigation. 

  1. Click the following link or paste it into your browser: https://applicationstation.certiphi.com
  2. Click the "SIGN UP NOW" button to create an account.
  3. Enter the Code: USFMCOMCBC in the Application Station Code field.
  4. Follow the instructions on the Application Station web site.

Note – please store the username and password created for Application Station in a secure location.  This information is needed to enter Application Station in the future which includes obtaining a copy of your background investigation report. 

If you encounter issues with the Application Station: Student Edition or have questions regarding the site, please contact Certiphi Screening’s Help Desk at 888-276-8518, ext. 2006 or itsupport@certiphi.com.

Background Investigations are completed, on average, within 3 to 5 business days.  Once completed, you will receive an email from Certiphi Screening, studentedition@certiphi.com. Follow the link in the email to access Application Station: Student Edition to view the report. To access the site use the same username and password created at the time you submitted your background check.  Application Station includes instructions for disputing information included in the background check should you feel anything is incorrect. 

The background investigation consists of the search components listed below.  All records are searched by primary name and all AKAs, a student’s primary address, and all addresses lived within the past three years. 

  • Social Security Number Validation and Verification
  • Florida Statewide Criminal Search
  • County Criminal Records Search
  • National Criminal Database Search
  • National Sexual Offender Registry Search
  • SanctionsBase Search
  • OIG/EPLS Search

The cost of the Background Investigation is $56.25.  Certiphi Screening accepts credit cards and PayPal.  Payment is collected within Application Station: Student Edition. 

Drug Screening

Below are step-by-step instructions for accessing Application Station: Student Edition to authorize and pay for a drug screen, as well as locate a specimen collection site. Drug screen collection facilities are listed on the final page of Application Station: Student Edition.

  1. Click the link below or paste it into your browser: http://www.applicationstation.com
  2. Enter the Code: USFMCOMDRUGSCREEN in the Application Station Code field.
  3. Click the "SIGN UP NOW" button to create an account.
  4. Follow the instructions on the Application Station web site.

If none of the collection sites listed are convenient (within 30 minute drive), please contact Certiphi Screening’s Occupational Health Screening Department (i.e. TriTrack and Scheduling Hotline) for assistance with locating an alternate location; phone number 800-803-7859.

All drug screens conducted for University of South Florida Morsani College of Medicine and School of Physical Therapy and Rehabilitation Sciences are 10-panel and tests for:

  • Amphetamines
  • Cocaine Metabolites
  • Marijuana Metabolites
  • Opiates
  • Phencyclidine
  • Barbiturates
  • Benzodiazepines
  • Methadone
  • Propoxyphene
  • MDMA/Ecstasy

You will receive an email from Certiphi Screening, studentedition@certiphi.com, once drug test results are available.   Follow the link in the email to access Application Station: Student Edition to view the report. 

The cost of the Drug Screen is $29.00.  Certiphi Screening accepts credit cards and PayPal.  Payment is collected within ApplicationStation: Student Edition. 

HIPAA/OSHA (Safety: Back to Basics)

Obtain proof of your HIPAA/OSHA training at https://learn.health.usf.edu/login/index.php.

For "HIPAA" training, it depends on whether that means just the HIPAA module or whether that's referring to the "Professional Integrity" training as a whole (HIPAA is part of the Professional Integrity training). The HIPAA module does have its own certificate. To access the certificate for just the HIPAA module:

1. Log in to USF Health Learn at: https://learn.health.usf.edu

2. Click "USF Health Training" under Course Categories.

3. Click "USF Health Professional Integrity & Compliance."

4. Complete the "HIPAA Privacy at USF Health" module, if not already completed.

5. Click "HIPAA Privacy at USF Health Completion Certificate" (listed just below the module).
