USF DPT Offer Letter - Documents

It is necessary that you inform the School of Physical Therapy & Rehabilitation Sciences, in writing on the form below, of your decision concerning acceptance of this offer of admission.  This form must be received no later than ten business days from the date of receipt of this letter or your position in the DPT Class of 2018 will be offered to another student. Please mail your reply to the address on the form via certified, registered return–receipt US mail for tracking purposes.

Students who require accommodations will be asked to visit the Morsani College of Medicine School of Physical Therapy & Rehabilitation Sciences to discuss their needs prior to matriculation.
