It is necessary that you inform the School of Physical Therapy & Rehabilitation Sciences, in writing on the form below, of your decision concerning this offer of Alternate for Admission and placement on our Waiting List. This form must be received no later than ten business days from the date of this letter or you will forfeit your position as an Alternate for the DPT Class of 2018. Please mail your reply via certified, registered return–receipt US mail for tracking purposes.
